Important Patient Notice

From 01 October we will be changing the provider of our online consultations software from Klinik to the North West London provider called Patches. Following the successful merger of our clinical systems we are now able to move to another supplier and North West London have identified Patches as the best solution for patients.

There will be no change to how you access our services and you will still use Patches for new health problems or health problems that have gotten worse via a link on our website or texted to you and if you are unable to access online services a member of our team can complete the form on your behalf.

Patches also helps patients who have hearing or speaking impairments, anxiety, and who speak English as a second language. Please be aware you will need to use either the NHS app or a Patches log in to access us.

Further information will be available on our website from 01 October 2024.

Important Patient Notice

On the 01 April 2024, Grove Park Surgery, Wellesley Road Surgery and Chiswick Family Doctors Practice have now merged to form Chiswick Medical Practice.

Our main site will be Grove Park Site at Burlington Lane and we will continue to deliver services at Wellesley Road site (previously Wellesley Road Practice) and Chiswick Health Centre Site (previously Chiswick Family Doctors Practice).
