Understanding your local NHS: How it all fits together and understanding the acronyms

At Chiswick Medical Practice, we want to help you understand how healthcare services are organised in our area and what they mean for you.

Who’s who in local NHS care

Integrated Care Board (ICB)

The NHS North West London Integrated Care Board (ICB) plans and pays for most NHS services in our area. It looks after everything from hospital care and mental health to GP surgeries and community services.


We are part of this wider system in North West London.

Primary Care Network (PCN)

We are part of the Chiswick Primary Care Network (PCN) – a group of local GP practices working together to offer better services to patients.

Our PCN includes:

  • Chiswick Medical Practice
  • Chiswick Health Practice
  • Glebe Street Surgery
  • Grove Park Terrace Surgery
  • Holly Road Medical Centre
  • West4GPs

Working together means we can offer more joined-up care closer to home.

What the PCN Offers

Through a national NHS scheme called the Direct Enhanced Service (DES), our PCN provides extra services by hiring different health professionals using the ARRS (Additional Roles Reimbursement Scheme). These include:

  • Pharmacists – for medication reviews
  • Social Prescribers – to connect you with local support and services
  • Mental Health Workers – for emotional and psychological help
  • Physician Associates & Paramedics – to help with assessments and treatment

The Bigger Picture: Hounslow’s Care System

We’re part of the Hounslow Borough Based Partnership – a local team of health and care providers working together as part of the North West London Integrated Care System.

This team includes:

  • GP practices (like us, in PCNs)
  • Community health services (e.g. district nurses, therapists)
  • Mental health services
  • Leisure services
  • Social care (run by Hounslow Council)
  • Voluntary and charity organisations

These groups work together so you get the right care, at the right time, in the right place.

They look at projects like

  • Frailty and preventing falls
  • Preventing health inequalities
  • Special educational needs and complex needs, mental health and children’s therapies
  • Integrating health and social care
  • Supporting digital
