What Is Extended Access?
Extended Access appointments give you the opportunity to see a GP, nurse, or other healthcare professional outside of normal surgery hours, including evenings and weekends. This service is designed to make it easier for you to access care if it’s difficult to attend during the day due to work, school, or caring responsibilities.
Extended Access at Chiswick Medical Practice is delivered on behalf of Chiswick Primary Care Network (PCN). Chiswick Medical Practice provides around 50% of all Extended Access appointments for patients across the PCN, helping to ensure more flexible and timely access to healthcare locally.
Our Commitment to You
You should receive the same high standard of care during an Extended Access appointment as you would from your own registered practice. Clinicians working in Extended Access can securely access your medical record, update your notes, and arrange tests or referrals in the same way as your usual GP.
When and Where Are Appointments Available?
- Weekdays: Monday–Friday, 6:30 pm–8:00 pm
- Weekends: Saturday, 9:00 am–5:00 pm (Times may vary slightly by site – please check when booking.)
You may be seen at a different local surgery, but your medical record and any test results will automatically be shared with your registered GP practice.

How to Book
- Call your usual GP practice during normal opening hours
- Submit an online request via your practice website
- Ask at reception for an “Extended Access” appointment
Extended Access appointments are booked in advance; this is not a walk-in service.
Patient Information: Referrals and Tests
Two-Week-Wait (2WW) Referrals – Fast Track Cancer Referrals
If your doctor refers you under the Two-Week-Wait (fast track) pathway, you should be offered a hospital appointment within two weeks. If you do not hear from the hospital within this time, it is very important that you contact your registered GP practice to let them know. They can check the referral and follow up with the hospital on your behalf.
Other Referrals
If you are referred for another type of specialist appointment (not fast track), the referral will be sent to your registered GP for processing. Once this has been done, any questions or follow-up about your referral should be directed to your registered GP practice, not the Extended Access team.
Blood Tests
If you have a blood test during your Extended Access appointment, your results will be sent to your registered GP. They will contact you if any follow-up is required.
If the Extended Access doctor advises you to have a blood test but it is not carried out during your appointment, you will need to book this through your registered GP practice. The Extended Access staff are unable to arrange blood test appointments for you.
Imaging (X-ray or Ultrasound) Requests
If you are asked to have an imaging test (for example, an X-ray or ultrasound), the extended hours doctor will send the referral and the hospital will contact you directly with an appointment date. Your results will be sent to your registered GP for review and follow-up.
If you have any queries about your appointment or test or wish to chase results, please contact your registered GP practice directly.
If you have not heard within the timeframe given at your appointment, contact your practice for an update. If your symptoms worsen, please seek advice via NHS 111 or attend A&E if necessary.
