CHISWICK MEDICAL PRACTICE SHORTLISTED FOR THE 2025 HSJ AWARDS
SETTING THE STANDARD FOR HEALTHCARE EXCELLENCE

Chiswick Medical Practice is proud to announce that it has been shortlisted for three categories at the prestigious 2025 HSJ Awards:

  • Primary and Community Care Provider of the Year for Leading the Way: A Blueprint for Scalable, Sustainable General Practice
  • Digitising Patient Care Award for Right Care, Right Place, Right Time: A Patient-Centred Digital Hub
  • Patient Safety Award: Primary Care initiative of the Year forRight Care, Right Place, Right Time: A Patient-Centred Digital Hub

This recognition highlights the exceptional work and innovation demonstrated by the team and is a testament to Chiswick Medical Practice’s dedication to driving healthcare excellence.

This year’s awards attracted more than 1,250 entries from across the health sector. With only 245 projects and individuals making it through to the final shortlist, the competition has been incredibly tough, demonstrating the impressive scale of talent, commitment, and impact across the NHS and wider healthcare landscape.

Chiswick Medical Practice’s shortlisted entries were selected following a rigorous judging process and have been recognised as standout examples of excellence in healthcare delivery.

The 2025 awards judging panel was once again made up of a diverse range of highly influential and respected figures within the healthcare community, including: Matthew Style, Director General, Secondary Care and Integration, Department of Health & Social Care; Mark Cubbon, Chief Executive Officer, Manchester University NHS Foundation Trust; and Amanda Doyle, National Director for Primary Care and Community Services, NHS England, as well as a range of esteemed Chief Executives from NHS Trusts across the UK.

  • For Primary and Community Care Provider of the YearLeading the Way: A Blueprint for Scalable, Sustainable General Practice showcases how Chiswick Medical Practice has merged four local surgeries into a single, unified service — creating a scalable, patient-centred model that is sustainable for the future of general practice.
  • For Digitising Patient Care AwardRight Care, Right Place, Right Time: A Patient-Centred Digital Hub highlights the practice’s pioneering work in implementing an AI-enabled workflow system that ensures patients are directed to the right clinician or service, improving access, efficiency, and patient outcomes.

Winners will be announced at the awards ceremony on 20th November 2025 at Evolution London. The event celebrates excellence in healthcare, promotes best practice, and recognises the people and projects improving outcomes for patients nationwide.
